Open Enrollment for 2010


  • Open Enrollment is Dec. 1-14, 2009
  • Coverage will be effective January 1, 2010



DATE:   NOVEMBER 25, 2009

The annual open enrollment for voluntary life insurance will be held December 1, 2009 through December 14, 2009 and this year we are able to offer a little better option. You may enroll for voluntary supplemental life insurance WITHOUT evidence of insurability under the following guidelines:

  • All employees are eligible to enroll in the Optional Life Coverage up to a modified Guarantee Issue of $100,000*
    • Spouses may enroll up to a GI of $30,000*, and
    • Children are a Guarantee Issue of $10,000*
  • Employees and Spouses From Age 60-age 69 are only eligible for a Guarantee Issue of $25,000 (subject to age reductions)
  • Employees and Spouses over age 69 are not eligible to participate in the Modified Open Enrollment Offering.
  • Anyone electing coverage over the Modified Guaranteed Issue Amounts will be subject to providing Evidence Of Insurability

*Anyone previously Declined will not be eligible to participate in this Open Enrollment Offering.

Coverage will become effective January 1, 2010.

The completed forms must be returned to the Human Resources Department no later than 5:00 PM on December 14, 2009. Forms will also be available at each of the 3 open enrollment meetings.
